• APPENDIX-1: Athlete CV and List of Achievements (To be Written by the Candidate Himself)
• APPENDIX-2: Club Reference Letter (To be Written and Approved by the Final (Last) Club)
• APPENDIX-3: Coach Reference Letter (To be Written by the Last Coach)
• APPENDIX-4: Nationality Certificate to be obtained from the Federation (To be obtained from the Federation of the Branch)
• APPENDIX-5: License Photocopy (Photocopy of the Last License with Visa)
