Treatment options for Demodex-related eyelid inflammation were scientifically compared

Faculty members of the Istanbul Medipol University School of Medicine, Prof. Göktuğ Demirci and Prof. Sevil Karaman Erdur, conducted a scientific study comparing treatment options used against the eyelash follicle parasite that causes eyelid inflammation, identifying the fastest and most effective methods. The study was published in Contact Lens and Anterior Eye.

Professors Göktuğ Demirci and Sevil Karaman Erdur from Istanbul Medipol University Faculty of Medicine have conducted a scientific study comparing different treatment options for the Demodex folliculorum parasite, which can settle at the base of eyelashes and cause eyelid inflammation (blepharitis). The study, titled “Comparative in vitro efficacy of ivermectin, lotilaner, lime sulphur, tea tree oil, and lemongrass oil against Demodex folliculorum” was published in Contact Lens and Anterior Eye, a prestigious international journal in the field of ophthalmology ranked in the Q1 category.

The Demodex parasite may cause symptoms such as itching, redness, a stinging sensation in the eyes, and dandruff-like scaling at the base of the eyelashes. Although it is relatively common in the general population, diagnosis is often delayed because its symptoms can be confused with those of other ocular diseases. Persistent scaling at the eyelash base, chronic redness, and recurrent eyelid inflammation are among the most distinctive clinical signs of

Demodex infestation. For this reason, Demodex-related eyelid inflammation has gained increasing importance in both clinical practice and scientific research in recent years. 

AIM OF STUDY: COMPARING EXISTING TREATMENTS AND EMPHASIZING SAFE USE 
The study compared the in vitro effectiveness of medical agents currently used in Demodex treatment, as well as certain natural substances. The primary aim was to scientifically determine how quickly, and effectively different active compounds act on the parasite and to provide comparative data on existing treatment options.

Within this framework, the effects of medical agents such as lotilaner and ivermectin were evaluated alongside natural or semi-natural substances including tea tree oil, lemongrass oil, and lime sulphur. Thus, both modern medical treatments and commonly used natural substances were compared for the first time under identical experimental conditions.

Live parasite samples were observed under a microscope, and the effects of each agent on parasite motility were assessed under standardized conditions. In this respect, the study makes a significant contribution to the literature by examining different approaches used in Demodex treatment within a unified scientific framework for the first time.

WHAT DO THE FINDINGS INDICATE? 
The results demonstrated clear differences among the tested agents in terms of both onset of action and overall effectiveness. While some medical agents acted more rapidly on the parasites, certain natural substances were also found to produce measurable effects under laboratory conditions. In particular, the rapid immobilization of parasites by lemongrass oil and the strong performance of lotilaner stood out among the key findings.

However, the researchers emphasized that these results were obtained under controlled laboratory conditions and should not be interpreted as direct clinical treatment recommendations. The findings provide a scientific basis for improving treatment approaches, but further comprehensive clinical studies are required before translating these results into routine clinical practice.

NO SUBSTANCE SHOULD BE APPLIED TO THE EYES WITHOUT SCIENTIFIC EVIDENCE  
The researchers underlined that the eye is an extremely sensitive organ and stressed that no substance whose safety and appropriate dosage have not been scientifically demonstrated should be applied to the eye. Products recommended through social media, hearsay, or non-medical sources may cause permanent and irreversible damage when used without professional guidance.

It was also emphasized that the effectiveness of natural substances in laboratory settings does not imply that they can be used in an uncontrolled manner. Individuals suspected of having Demodex-related eyelid inflammation are strongly advised to consult an ophthalmologist rather than resorting to self-treatment methods.

The research team stated that the study not only provides a scientific evaluation of the effectiveness of existing treatments but also highlights that natural or alternative approaches must be addressed exclusively through controlled, ethical, and evidence-based scientific research. Such studies are expected to contribute to the development of more effective, safer, and lower-risk treatment strategies in the future.
